Privacy and data use
A plain-language explanation of what happens to the information you provide with an appointment request.
Information collected
Your name, phone number, optional email address, selected appointment type, requested date and time, and any short scheduling note you add. A one-way technical identifier derived from your connection address is kept briefly to limit abuse.
How it is used
To check availability, confirm the appointment or suggest another time, send status messages, contact you, and keep the operational records needed to manage the request.
Collection and basis
Information is collected electronically through this form and processed only as needed to manage the appointment request and the related planning of healthcare services.
Service providers and transfers
Your information is not made public. Technical providers such as Cloudflare may process data only as needed to provide website, database, and email infrastructure. When the contact-page map loads, OpenStreetMap servers may receive technical connection details; choosing the directions link takes you to Google Maps. Disclosures required by law to authorised bodies remain possible.
Retention and deletion
Information is kept only for as long as needed to manage the appointment process and meet applicable legal obligations. It is deleted, destroyed, or anonymised when the conditions for processing no longer apply.

You can use the channels on the contact page for requests concerning your rights under Article 11 of Turkey’s Personal Data Protection Law No. 6698.
Cookies, measurement, and advertising
Optional measurement and advertising tools are disabled by default. If configured, they run only after the visitor makes a choice, which can be changed through the “Cookie preferences” link in the footer.
